The FBI responded to the developing standoff with its' elite Hostage Rescue Team, but the FBI on-scene leadership failed to critically assess the situation and the conflict escalated. The failure of the FBI leadership to properly assess the situation and critically analyze their actions resulted in the death of Vicki Weaver. FBI leaders failed to critically analyze initial situation reports, establish appropriate rules of engagement and develop a sound plan to end the standoff.
